Care Management
At the center of all Triangle Care Management services is a dedicated, one-on-one Care Manager (CM) who both assesses your needs in a welcoming, affirming environment and educates you on the full-range of assistance available through both internal resources and external referrals. After meeting with a CM, you will be connected with appropriate individuals or services having expertise in addressing your specific needs as identified by the CM.
Benefit assistance (Medi-CAL, VA benefits, etc.)
Legal resources (estate planning, Do Not Resuscitate document, POLST, fiduciary)
Residential options (independent/assisted living, memory care, skilled nursing)
Transportation (non-emergency medical appointments, necessities, pharmacy, socialization)
Advocacy (In addition to facilitating interaction with services, the CM ensures clients do not “fall through the cracks” or get sidelined by bureaucracy.)
Assistive technology resources (safe house modifications, digital assistance, mobility devices , etc.)
Counseling (affiliated practices, LGBTQ centers, private practitioners)
Socialization (senior centers, LGBTQ centers, assistance in enhancing one’s own personal network of friends, neighbors, loved ones)
Help at home (CA-IHSS, Meals on Wheels, shared housing, light housekeeping, medication management, etc.)
Healthcare services (county health departments, local medical groups)
